Care Coordination
Care Coordination focuses on providing services and outreach for persons chemically addicted to substances. The effort is aimed at providing single point of accountability for service coordination and early engagement with the person served and their natural supports. Services provided to the targeted population include, but are not limited to, evidenced based programs such as High-Fidelity Wraparound, WRAP planning, Peer Support, and Motivational Interviewing.
All transfers of treatment services follow protocol for warm hand-offs with coordinating services for the proper level of treatment. The Care Coordination component also collaborates with the Detoxification Unit, Outpatient Services locally and within our catchment counties, our Residential Treatment facility, and other service providers.
We, at CARE, believe that chemically-addicted individuals are entitled to be treated with dignity and respect, ensuring confidentiality guidelines are followed at all times. We, at CARE, also believe that family involvement and family education is a vital element of recovery from chemical addictions and is an important element in alleviating the stigma associated with the treatment of addictions.
